Jake (Dr. Frazier)
DMD MS
Dr. Frazier was born in 1984 and grew up just outside of Mt. Vernon, IL. He graduated from Mt. Vernon Township High School in 2002. He then attended Rend Lake College where he received two associates degrees. He received a Bachelors Degree from Southern Illinois University in Edwardsville in 2007. He majored in Biology, with a specialization in Medical Science and a minor in Chemistry. He attended Southern Illinois University School of Dental Medicine from 2007-2011, earning his Doctorate in Dental Medicine Degree.
Dr. Frazier practiced General Dentistry in Southern Illinois from 2011-2013. During his time as a General Dentist, he decided that he wanted to specialize in Endodontics. In 2013, he was accepted into Saint Louis University's Endodontic residency and began the two year post-graduate program. He graduated in 2015 with a Certificate in Endodontics as well as a Master's Degree in Endodontics.
He has practiced Endodontics in Southern Illinois since graduation from SLU and decided to open his own practice in 2017.
Dr. Frazier spent several years on staff at Southern Illinois University School of Dental Medicine and as an adjunct assistant professor at Saint Louis University. He served as President of the St. Clair Dental Society, and is a member of the ISDS, ADA, and AAE. He enjoys giving presentations and mentoring students who are interested in dentistry or endodontics. He attends several meeting and conferences each year to stay current and continue his education.
Dr. Frazier worked at Applebee's from 2003-2008, where he met his wife Cindy (who worked there as well). They now have four young children and live in Lebanon, IL. If he is not at work, Dr. Frazier is spending time with his family. His hobbies include playing cards, reading, anything outdoors, and restoring/collecting 80s arcades games and 90s pinball machines.

Amber has many years of dental assisting experience. She is a perfectionist and an extremely hard worker. She loves spending time with patients and is great at helping people feel comfortable during their treatment visit. When she is not working she enjoy's spending time with her husband and two adorable young children. They love crafting, boating, and enjoying the great outdoors!
Cassie
Dental Assistant
Cassie has over 7 years of experience in the medical field and is always upbeat and compassionate. She takes a lot of pride in everything that she does and loves getting to know patients during their visits.
When she in not in the office, she is spending time with her wonderful husband and three young daughters. They spend their time at their family clubhouse, boating, and enjoying time in the sun.
Jamie
Office Manager
Jamie has been involved in dentistry since she was a teenager. She is wonderful at her job and takes pride in helping our patients. She is an expert when it comes to insurances, and she is here to make sure that you understand your coverage and maximize your benefits. Jamie is here every day to answer any questions you may have about treatment, billing, or our office. She and her husband have a little boy, and enjoy spending time together outdoors, boating, or in the bowling alley sharpening their skills!
